TRANSCRIPTS / RECORDS REQUEST
Please read the entire page before filling out your form and sending it in. Following the directions will save you time and expense.
OFFICIAL CALIFORNIA TRANSCRIPTS – $40
OFFICIAL OUT-OF STATE TRANSCRIPTS* – $125
WE WILL MAIL YOUR OFFICIAL TRANSCRIPTS TO THE CAMTC AND ALSO MAIL YOU A COPY. Please let us know if you need a copy sent to FSMTB at the same time.
ALL transcript requests and payments must be issued and submitted by the
*Transcript fee for transcripts to be sent to Federation of State Massage Therapy Boards for the purpose of taking the MBLEX test are $40.00, even though FSMTB is located in Tennessee. This is subject to change if they have additional or different requirements, beyond that of CAMTC.
Lauren McLachlan is the designated Custodian of Record for Healing Arts Institute.
Transcripts requests must be sent via US Postal mail with a check or money order made payable to Lauren McLachlan
ALL contact is to be done via the US Mail. Period. There is no location to visit. There is no phone number to call. Healing Arts Institute no longer exists.

Read all the directions before filling out the form. Check to ensure you have followed them before sealing your envelope or it will create a delay in processing your request.
You can leave the student ID number blank.
Please include your birth date on the transcript request form.
If you have already been certified as CMP or CMT with CAMTC, please put your CAMTC number on your transcript request.
Under the section TRANSCRIPT RECIPIENT INFORMATION please list the person or entity you want to receive your transcript. Where it is to be mailed.All transcripts are automatically sent as a copy to you. Please use this section to designate who else you want to have your Official Transcript sent to…. CAMTC, another state’s massage board, a school (not Healing Arts Institute- that is listed under school information as the entity which creates your transcript.)
Under the section STUDENT INFORMATION it is required to list the physical location, street address, city, state and zip code. Addresses supplied for PMB (Personal Mail Box) cannot be used for the physical location requirement. Mailing address using a PO Box can be put in addition to your physical address.
